I believe that each gym is unique and that everyone's needs vary; to help support your program best I've prepared a  questionnaire below that will assist in providing the most accurate information possible.

Please be sure to fill it out as accurately and as thoroughly as you can. If you don't know the answer to any of the required questions, simply input "N/A" or "Not sure yet." You know, something to that effect ;D
